Nova Records Management (NOVA) is an Information Management Company specializing in serving the release of information needs of private and group medical practices. Currently, we assist more than 5,000 healthcare, insurance and legal professionals with timely, authorized and efficient release of patient medical records.

In order to start the Request for Medical Records process, please download, print and return a valid Authorization Form. Click on the button to the left to download this form (pdf file).

If you wish to pay for records by credit card, please complete and fax us a Credit Card Authorization.

NOVA offers an online service that allows authorized users to check the status of their requests. No medical records are available via this service. If you are a requester (patient, attorney, insurance company, etc.) click on the Requester Login button to begin a NOVA Online session. You can then check the status of your request, check the status of an invoice and contact NOVA regarding any of your outstanding requests.

NOVA also offers an online service to its providers that allow them to check the status of requests by patient or requestor. If you are a provider of medical records and a client of ours, please click on the Provider Login button to start your online session.
